Giggle and Grow Childcare & Preschool

Enrollment

Prior to your child's start date the following must be returned to Giggle and Grow Childcare & Preschool:

• Completed Emergency Information Form, with current immunization records
• Parent Agreement
• Rapid Tuition Enrollment ACH Form (complete either this ACH or the below Credit Card form)
• Rapid Tuition Enrollment CC Form (complete either this Credit Card form or the above ACH form)
• KidsVision Authorization Form
• “Getting To Know You” Information Packet
• Security Door Form
• Parent Release Form for Media Streaming/Recording
• Infant Supplemental Information (if enrolling an infant)

Please keep us informed of any changes in medical or emergency information. In order to maintain compliance with the Arizona Department of Health Services and Childcare Licensure we must maintain current and accurate records at all times.
